To practise medicine in the UK, you first have to be issued a licence by the GMC (General Medical Council).

After that, Appraisal is the requirement that all licensed doctors have their medical practice formally reviewed by an Appraiser annually. To keep your medical licence in the UK, you will have to go through Annual Appraisal approximately once every 12 months. Your Annual Appraisal is part of the GMC’s Revalidation framework – one leads to the other.

For your Appraisal, you will meet with a qualified and approved Appraiser. If your Appraiser recommends that you should keep your licence, the GMC will most likely uphold that recommendation. It is for the GMC to decide whether you will keep your licence or not.

How is Annual Appraisal linked to Revalidation by the GMC?

Revalidation is the GMC’s mechanism to determine whether you continue to be fit to practise medicine in the UK. Every licensed doctor has to Revalidate approximately every five years during their career.

Revalidation affects all licensed Doctors, whether with or without a connection to a Designated Body or Suitable Person. Those without are usually Doctors who kept their licence for the last few years while working outside the UK only.

So, if you are employed by the NHS, your Revalidation will likely be simple and painless for you.

Your Annual Appraisal is connected to Revalidation. For every year leading to your Revalidation, it’s your Annual Appraisal that will produce the essential outputs to determine whether you remain on track for a successful Revalidation at the end of your five-yearly career cycle.

Going through Revalidation from outside the NHS will include a formal assessment of your knowledge by the GMC. This is a written multiple choice test customised by specialty. Alternatively, if you complete fully one of these assessments during the last five years prior to your Revalidation, you won’t need to take the GMC’s own Revalidation assessment.

All things considered, we think that for most doctors taking part in Annual Appraisal and Revalidation from outside the UK is not worth the investment in resources. You would likely be better off doing more constructive career moves, namely:

  • the digital platform cost. If your NHS employer has given you free access to an Appraisal platform, this will be zero.
  • the Appraiser’s fee. If you are employed in the NHS, this will cost you nothing.
